For your child’s first appointment, please bring any insurance information with you. Do I Need to Bring my Insurance Card?

Arizona Health Care Cost Containment System (AHCCCS)
Islands Pediatric Dentistry is preferred provider with the Arizona Health Care Cost Containment System (AHCCCS). If you would like Dr. Rez to be your little one’s “main dentist,” give Islands Pediatric Dentistry a call, we can make sure Dr. Rez is the dentist assigned to your children through the Arizona Health Care Cost Containment System program. Green Sky & CareCredit Flexible Financing
For those interested in financing their dental care, Islands Pediatric Dentistry is proud to accept both Green Sky and CareCredit for those who qualify. With both CareCredit and Green Sky, patients can finance 100% of their dental treatment while paying it monthly. Patients can expect no upfront costs, annual fees or even pre-payment penalties. Both offer various payment plans, allowing you to find one that works best for you and your family.
In-House Dental Savings Plan
As a courtesy to our patients, we offer an “In Office Savings Plan” to our patients without dental insurance, or with an insurance plan, we are not contracted with. The benefits of the “In Office Savings Plan” include:
- No Maximum Benefit
- No waiting period (Treatment can be started immediately)
- No deductibles
- No claim forms
- No pre-authorizations
- No ID card required
The Plan Covers:
- 2 Exams per year
- 2 sets of x-rays (if/as needed)
- 2 cleanings
- 2 fluoride treatments
